Mrs O was working restocking shelves at a well known supermarket. The stock cage which she was working from had been stacked in an unstable and inherently dangerous fashion. In particular the lowest crate of the stack did not fit in the crate above it securely, causing the stack to be unstable. The crates toppled over causing damage to Mrs O’s right leg.
